System Transformation Grant Evaluation

The Michigan Department of Community Health (MDCH) Office of Long Term Care Supports and Services (OLTC) received a System Transformation Grant (STG) to reform several aspects of Michigan's Long Term Care System. MPHI is contracted to design and implement an evaluation of activities conducted under the STG. The MPHI evaluation team participated in a strategic planning process designed to turn the objectives of the grant into actionable strategies and measurable outcomes. Through this process, the evaluation team gathered qualitative information regarding the measurement priorities of stakeholders and offered input into the identification of outcomes. Based on the ideas generated by stakeholders during the strategic planning process, the evaluation team drafted an initial evaluation plan, which was reviewd by a stakeholder group convened by OLTC. The evaluation team will implement the evaluation over the next several years. Stakeholder participation will continue to be a critical component of the evaluation, and part of the evaluation teams role will be to elicit and incorporate stakeholder feedback. In order to ensure that the evaluation plan is consistent with STG activities, the evaluation team will monitor the implementation of the strategic plan. The evaluation team will work closely with MDCH staff, participate in ongoing strategic planning, and hold discussions with stakeholders as needed to assess the extent to which the strategic plan is implemented as designed.
