The purpose of this project was to develop and implement a process and outcome evaluation of MDCHs Primary Care Initiative (PCI), a system reform initiative designed to improve primary care services in the State of Michigan with a specific focus on reducing health risks such as obesity. MPHI has worked in partnership with PCI to develop a logic model and an evaluation plan that was feasible, methodologically rigorous, and designed to answer questions of importance to the initiative. Once the evaluation plan was finalized, MPHI worked in partnership with PCI and other stakeholders to coordinate the implementation of the evaluation with the implementation of the initiative.
