The objective of this project was to support the Michigan Department of Community Health (MDCH) in its efforts to educate, train and provide technical assistance to long-term care (LTC) providers on eligibility and access to services for Medicaid recipients using a new online LTC determination screening tool, the Level of Care Determination (LOCD). Regional meetings were held to train approximately 700 LTC providers and hospital discharge planners on the new Medicaid policy and use of the LOCD. MPHI-CDMTR established a Helpdesk to provide ongoing technical assistance to LTC providers on use of the state of Michigan single sign-on (SSO) portal and the LOCD.

Beginning November 1, 2004, MDCH implemented a new online LTC determination screening tool.
